Billing Notice
Our Process
For All Office Exams, Consults and Procedures payment is expected at time of service and will be collected. We can bill your insurance as a out of network provider or give you a bill to send in to your insurance for reimbursement from your plan. We also have billing consultants who can help you with this . You should understand what your plan covers, if you have out of network coverage, your copay and deductible and understand that you are responsible for the services if insurance does not cover.
When coming in for your appointment we will take a copy of your current insurance card front and back and a photo ID . Payments we accept are cash,check,credit/debit cards and direct banking payments.
Telehealth
We Offer Telehealth Packages
Telehealth (also called telemedicine) allows us to provide care for you without an in-person office visit. All you need is access to the internet on your computer, tablet, or smartphone!
There are several options for telehealth care:

Send and receive messages from us only using secure messaging, email, secure messaging, and secure file exchange.
Health Documents
Required Forms
All patients new to Abundant Wellness LLC. are required to complete a Medical/Gyne History form, HIPPA, and Email Transmission form. If you want your records reviewed prior to your appointment, complete Records Release form and send to your provider.
Bring the completed forms with you to your appointment or email them to info@abundantwellnessco.com. Also bring a photo ID, insurance cards, method of payment. We accept cash, credit cards or directed bank payments.
